I must have the following completed form with all signatures before I can treat your animal. You may download it below and bring it with you to the first visit, or email me the form ahead of time.

Equine and Other Large Animals:
Initial Exam & Adjustment: $250
Adjustment: $200
Small animals:
Initial Exam & Adjustment: $80
Adjustment: $50
